Viagra (Sildenafil) by Pfizer will probably become one of the major pharmacological advances in the 19th Century. This Type 5 phosphordiesterase inhibitor is successfully treating erectile dysfunction in more than 75 percent of the cases in men. Now some urologists are prescribing a new delivery system for Viagra, a compounded version that is used sub-lingually, that is, under the tongue.
The Viagra in pill form has a bitter alkaloid taste, slow absorption from the gastrointestinal tract, which sometimes could produce an unpredictable absorption rate. Better absorption can occur on an empty stomach of at least two hours, which could mean more careful planning would be needed for sexual activity. It takes 30 minutes for the drug to begin absorbing and assuming the stomach is empty should reach his peak in one hour and maintain it's action maximally for up to four hours. Food contents, on the other hand, will slow the absorption and may require two plus hours before one reaches maximal levels in the blood and therefore for some men maximal abilities to become erect.
If a physician has a relationship with a patient and a prescription to compound a drug in a new format or delivery system is sent to a compounding pharmacist, a drug may be produced in this new format or delivery by a licensed compounding pharmacist. Patients can check with their health care providers about compounding pharmacists.
Urgent Care Pharmacy, which does compounding, in Spartanburg, South Carolina, has developed a new delivery system for Viagra. It involves taking Sildenafil produced by Pfizer, which has the patent rights to Sildenafil and mixing this drug into a proprietary dissolvable mint tasting sublingual tablet that when placed under the tongue is absorbed by the lining of the mouth and the tongue. Usually, then within 10 to 15 minutes, the drug is absorbed giving sufficient blood levels to allow for sexual stimulation to produce an erection in most cases. The advantages include short term planning of sex and no concern for stomach contents. Some urologists say the effects are similar to the Viagra tablet. However, the amount of medication necessary is much less since the drug is almost completely absorbed through the mouth and Viagra can be incompletely absorbed through the stomach. The sub-lingual come is 20 and 40 microgram doses. in 25, 50 and 100 micrograms, with most men being prescribed 50 or 100.
The positive and negative effects of the sublingual delivery system appears to be the same. These include mild headaches, facial flushing, stomach upset and the bright vision with a blue-green halo, but in most cases is insignificant, of short duration and frequently disappears as the medication is taken multiple times. Safety with the nitroglycerin is similar to the Viagra oral tablets and the patient should not be taking any form of nitrates or nitroglycerin which could be life threatening if taken within 24 hours of the Sildenafil.
For all practical purposes there are no other contraindications and the drug can be prescribed for patients who have heart decease, hypertension and heart irregularities. On the other hand, patients who can't tolerate sexual activity without chest pain should not be having sex and therefore should not be taking any form of Sildenafil whether it's oral Viagra or the sublingual delivery system. No person should take Viagra without an examination from their health care professional first.
Patients can discuss the sublingual delivery system with their physicians, who can then contact a licensed, qualified compounding pharmacist.
